This document was written around the time this site was created, for those who wish to understand what this site is about. And while our understanding of the world and events has grown since then, the basic ideas remain sound and true to the purpose.

So please post all links, thoughts and ideas that you feel will benefit the readers of this site to the comments under this post each day. And thank you all for your contributions.

  1. Similarities abound, but direct comparisons have limitations. Fiscal reform is inevitable as the western world seems destined for the same fate, only this time the debt levels are worldwide. Pandemic-created government dependents are already massive. Welfare state obligations are much greater. Pension obligations are beyond huge. Unionization in broad labour swaths will create large legal wrangling with little effect except for governments to pay in diminished fiat.

    A counterterrorism organization formed by some of the biggest U.S. tech companies including Facebook and Microsoft is significantly expanding the types of extremist content shared between firms in a key database, aiming to crack down on material from white supremacists and far-right militias, the group told Reuters.

    Until now, the Global Internet Forum to Counter Terrorism’s (GIFCT) database has focused on videos and images from terrorist groups on a United Nations list and so has largely consisted of content from Islamist extremist organizations such as Islamic State, al Qaeda and the Taliban.

    Over the next few months, the group will add attacker manifestos — often shared by sympathizers after white supremacist violence — and other publications and links flagged by U.N. initiative Tech Against Terrorism. It will use lists from intelligence-sharing group Five Eyes, adding URLs and PDFs from more groups, including the Proud Boys, the Three Percenters and neo-Nazis.

    The firms, which include Twitter and Alphabet Inc’s YouTube, share “hashes,” unique numerical representations of original pieces of content that have been removed from their services. Other platforms use these to identify the same content on their own sites in order to review or remove it.

    While the project helps combat extremist content on mainstream platforms, groups can still post violent images and rhetoric on many other sites and parts of the internet.

    The tech group wants to combat a wider range of threats, said GIFCT’s Executive Director Nicholas Rasmussen in an interview with Reuters.

    “Anyone looking at the terrorism or extremism landscape has to appreciate that there are other parts…that are demanding attention right now,” Rasmussen said, citing the threats of far-right or racially motivated violent extremism.

    The tech platforms have long been criticized for failing to police violent extremist content, though they also face concerns over censorship. The issue of domestic extremism, including white supremacy and militia groups, took on renewed urgency following the deadly Jan. 6 riot at the U.S. Capitol.

    Fifteen companies can access the GIFCT database, including Reddit, Snapchat-owner Snap, Facebook-owned Instagram, Verizon Media, Microsoft’s LinkedIn and file-sharing service Dropbox.

    GIFCT, which is now an independent organization, was created in 2017 under pressure from U.S. and European governments after a series of deadly attacks in Paris and Brussels. Its database mostly contains digital fingerprints of videos and images related to groups on the U.N. Security Council’s consolidated sanctions list and a few specific live-streamed attacks, such as the 2019 mosque shootings in Christchurch, New Zealand.

    GIFCT has faced criticism and concerns from some human and digital rights groups over censorship.

    “Over-achievement in this takes you in the direction of violating someone’s rights on the internet to engage in free expression,” said Rasmussen.

    The group wants to continue to broaden its database to include hashes of audio files or certain symbols and grow its membership. It recently added home-rental giant Airbnb and email marketing company Mailchimp as members.

    According to a new poll, the majority of Americans are pessimistic about the direction of the country at the sixth month mark of the Biden administration, a dramatic shift from the last time that the survey polled respondents on the matter.

    “A majority — 55% — of the public say they are pessimistic about the direction of the country, a marked change from the roughly one-third (36%) that said the same in an ABC News/Ipsos poll published May 2,” ABC News reported. “In the early May survey, Americans were more optimistic than pessimistic by a 28-percentage point margin. Optimism is now under water by 10 points. Looking ahead to the next 12 months, fewer than half — 45% — now report feeling optimistic about the way things are going, a significant drop from about two-thirds (64%) in the May poll.”

    TUNIS, July 25 (Reuters) – Police and protesters clashed in several Tunisian cities on Sunday as demonstrators demanding the government step down attacked offices of Ennahda, the moderate Islamist party that is the biggest in parliament.

    Witnesses said protesters stormed or tried to storm Ennahda offices in Monastir, Sfax, El Kef and Sousse, while in Touzeur they set fire to the party’s local headquarters.

    The violence came as hundreds of protesters rallied in each of the main cities after a spike in COVID-19 cases that has aggravated economic troubles and exposed the failings of a squabbling political class.

    The protests, the biggest in Tunisia for months and the biggest to target Ennahda for years, were called by social media activists. No political parties publicly backed the rallies.

    In Tunis, police used pepper spray against protesters who threw stones and shouted slogans demanding that Prime Minister Hichem Mechichi quit and parliament be dissolved. There were other big protests in Gafsa, Sidi Bouzid and Nabeul.

    The protests raise pressure on a fragile government that is enmeshed in a political struggle with President Kais Saied, as the government tries to avert a looming fiscal crisis amid a weeks-long spike in COVID-19 cases and increased death rates.

    The pandemic has hammered the economy which was already struggling in the aftermath of the 2011 revolution that ousted long-time authoritarian leader Zine al-Abidine Ben Ali.

    Public support for democracy has waned amid surging unemployment and crumbling state services.

    “Our patience has run out… there are no solutions for the unemployed,” said Nourredine Selmi, 28, a jobless protester. “They cannot control the epidemic … They can’t give us vaccines.”

    Last week, Mechichi sacked the health minister after chaotic scenes at walk-in vaccination centres during the Muslim Eid al-Adha holiday, where large crowds queued for inadequate supplies of vaccine.

    After a year of wrangling with Mechichi and the leader of Ennahda, Rached Ghannouchi, who is also parliament speaker, President Saied declared the army would take over the pandemic response.

    Some analysts saw the move as an attempt to expand his powers beyond the foreign and military role assigned to the president in the 2014 constitution.

    Ennahda, banned before the revolution, has been the most consistently successful political party since then and a member of successive government coalitions. It has backed Mechichi, a political independent, in his disputes with the president.

    “The message of anger was well received… but violence and burning are unacceptable,” said party official Maher Mahioub.

    Government paralysis could derail efforts to negotiate an International Monetary Fund loan seen as crucial to stabilising state finances but which could also involve spending cuts that would aggravate economic pain for ordinary people.

    Security forces involved in the raid said they were carrying out instructions from the country’s judiciary and asked all journalists to leave.

    Reporters said they were ordered by security officers to turn off their phones and were not allowed back into the building to retrieve their personal belongs.

    The officers confiscated other equipment.

    Celebrations broke out on the streets of Tunis late on Sunday after news broke out Tunisian Prime Minister Hichem Mechichi has been dismissed by President Kais Saied following a series of violent COVID-related protests across the country.

    Kais Saied also suspended the parliament after protesters flooded the streets of the capital where they had earlier clashed with the police.

    “We consider the interpretation made by President Kais Saied to be wrong and encountering the truth because he relied on some articles of the constitution that authorise the president to take exceptional measures, and when the president consulted me to take exceptional measures within the constitution, I considered that normal,” stated Tunisian Parliament Speaker Rached Ghannouchi.

    “However, we consider what happened to be the reality of a coup against the constitution, because it’s under a state of emergency, which obligates the Assembly of Representatives to be in continuous session. In this case, it is forbidden to dissolve the government, consequently, the president acted with a logic that takes some [fact] into account and does not take others,” Ghannouchi went on to say.

      Hundreds of anti-government protesters rallied outside the Ennahda Movement headquarters in Kairouan on Sunday, to mark Republic Day, the 64th anniversary of Tunisia’s independence.

      Protesters removed and burned the banner above the headquarters. They called for the dissolution of parliament and accused the government of worsening social, economic and health conditions in the country, especially in regards to the handling of the ongoing COVID-19 pandemic as cases of infections are rising.

      “In the past, 5 Dinars were enough for the whole day, today 5 Dinars won’t buy a tin of tomatoes. We demand that (President) Kais Saeed dissolves the parliament immediately,” said one local resident.

      Anti-government demonstrations also took place acrossed the nation in Nabeul, Sousse, Sfax and Tozeur.

    • Tunisia Leader Declares Curfew After Firing Government

      (Bloomberg) — Tunisia’s president ordered an overnight curfew and other restrictions, a day after his firing of the government and suspension of parliament triggered allegations of a coup.

      President Kais Saied banned movement of people or vehicles on the streets from 7 p.m.-6 a.m. until Aug. 27, while also prohibiting gatherings of more than three people in public roads or squares. His statement didn’t give a reason for the decision.

      Saied took action against parliament and his premier late Sunday after masses of largely young people demonstrated in the capital, Tunis, and other cities calling for the fall of the government and railing against hardships caused by the coronavirus pandemic. The moderate Islamist Ennahda party, the dominant force in parliament, accused the president of carrying out a coup.

Tunisia’s Jewish population is now the target of hostility and provocation from their own countrymen, a state of affairs for which the new president bears much responsibility….

        Jews lived in Tunisia for thousands of years in relative peace. Only 1,500 Jews remain in the country, most of them on the island of Djerba. They represent one of the last remaining Jewish communities in the Middle East outside of Israel.

        The president’s attitude has opened the door to antisemitism among the Tunisian population, which is growing increasingly commonplace. A preexisting hostility toward faraway Israel has turned into open hatred and acts of provocation against local Jews. In other words, anti-Zionism has revealed itself as antisemitism.

    President Joe Biden’s attempts to suspend or end border wall construction are costing taxpayers $3 million a day, with an estimated price tag of at least $1.8 billion, according to a new report authored by Senate Republicans.

    Biden on the day he was sworn in ordered his administration to halt construction and review whether to resume it. The halt was supposed to be for 60 days. Over 180 days later, no indication has been given on the results of the review and whether construction will resume.

    The California-based U.S. Court of Appeals for the Ninth Circuit on Friday ruled against Gov. Gavin Newsom’s coronavirus mandates that barred private school children from receiving in-person teaching.

    In a suit brought forward in July 2020 by the Center for American Liberty, 20 plaintiffs challenged an order by Newsom that barred in-person teaching in 32 counties – a mandate that affected 80% of California’s children.

    Madagascar said on Thursday it had foiled an attempt to assassinate various Malagasy figures, amongst which President Andry Rajoelina.

    Six people, two of them French nationals according to diplomatic sources, have been arrested.

    “According to all the investigations we have made, the people arrested are involved in the assassination attempt of the Head of State of Madagascar. The arrests followed all the norms. The arrests were not made by mercenaries but by the police,” Minister of Public Security Rodellys Randrianarison told the press.

    “Six people were arrested. Among them, two Malagasy are bi-nationals, there is also a foreigner and the others are Malagasy,” he added.

    Police said they intervene to seize weapons on Tuesday after monitoring the operation for months.

  11. CBC – Meet the unvaccinated: Why some Canadians still haven’t had the shot

    Some suspect the science. Some don’t think they’re vulnerable. And some just don’t trust government messaging.

    Canada’s vaccine campaign has been crushing it lately, with an impressive 80 per cent of eligible Canadians having had at least one dose of a COVID-19 vaccine.

    That statistic distracts from a troubling fact, however: more than six million Canadians still haven’t had a shot, just as experts are warning we need more coverage to beat back a possible surge of cases in the fall.

    The first-dose vaccination campaign now seems to be grinding to a halt, with fewer than 50,000 people getting a vaccine each day — down from a peak of over 185,000 last month — even though those doses are now readily available nationwide.

    CBC News has spoken to some unvaccinated Canadians to learn more about the hesitancy that has taken hold in some pockets of the country.

    Many of the holdouts say they’re concerned about safety and side effects. Others say they’re not happy with the current products on offer.

    There are also practical considerations. A number of the unvaccinated have a needle-related phobia that can make getting a shot a frightening experience. Some have severe allergies to the vaccine components. Some rural Canadians have had trouble with access.

    And experts also suggest somewhere between two and 10 per cent of the population is vehemently opposed to vaccines — no matter what public health officials say about the many benefits of getting a shot.

    Nadina Smith graduated from teachers’ college this spring and she’s feeling the pressure from family and friends to get a shot before school starts up in the fall.

    Smith, who is from Alberta, told CBC News she’s researched the science behind various COVID-19 vaccines and she’s most comfortable with the one-dose Johnson & Johnson shot, which uses the more conventional viral vector vaccine technology.

    Such vaccines use a modified version of a different virus (the vector) to deliver instructions to cells, and are widely used to prevent infectious diseases like influenza.

    New tech vs. old tech

    Canada ordered the J&J shot — 300,000 doses were delivered months ago — but there are no plans to use it as part of the vaccination campaign. Government officials have said the provinces and territories have shown no interest in obtaining this product.

    “I know the traditional vaccines aren’t rated quite as effective in the research — but I’m comfortable with that style. I would happily go at this very moment to get that,” Smith said.

    While the mRNA products produced by Pfizer and Moderna have been deemed safe and effective by Health Canada and other regulators after a careful review of clinical trial data, Smith said she’s still reluctant to accept a vaccine that was developed so quickly.

    She said she’s not opposed to vaccines (she describes herself not as “vaccine hesitant” but as “mRNA vaccine hesitant”) but she’s concerned about the possible long-term effects of mRNA shots in particular, which use relatively new technology.

    ‘I don’t want to be the guinea pig’

    “How do we know what kind of impact this is going to have on our bodies? Am I gonna have a third eye in 20 years?” she said.

    “I mean, I know I’m not gonna have a third eye, but I’m just trying to explain what I mean. We don’t know what the potential outcomes are in the long term.

    “The only thing that would have swayed me is if there was some sort of research or study of the long-term effects of COVID mRNA. For me, that is a huge concern and I don’t want to be the guinea pig.”

    Messenger RNA, or mRNA, directs protein production in cells throughout the body to trigger an immune response and protect against infectious diseases.

    While an mRNA vaccine has never been on the market until now, mRNA vaccines have been tested in humans for at least four infectious diseases: rabies, influenza, cytomegalovirus and Zika. No long-term side effects from those products have been reported.

    Researchers have been studying mRNA technology and its potential for three decades. With an injection of hundreds of millions of dollars in emergency funding from the U.S. government and other sources, companies like Moderna and BioNTech (and BioNTech’s partner Pfizer) turned a promising piece of molecular biology into a usable product that has been deployed in several hundred million people to great effect.

    Mixed messages

    Lorie Carty, a retiree from Prince Edward County, Ont., said the actions of the National Advisory Committee on Immunization (NACI) and Health Canada — two bodies that have sometimes offered competing advice about vaccines, most notably about the AstraZeneca product — have made her question the safety of the vaccines.

    “It seems like they’re flying by the seat of their pants, trying to figure things out as they go along and there’s just so much mixed information,” Carty said of federal health officials.

    She said she has an appointment booked but she keeps rescheduling because she’s just not ready to commit.

    “I want to be sure before I put that in my body because once it’s in there, there’s no going back,” Carty said.

    “I’m not saying I’m an anti-vaccine person. I just don’t have enough confidence. We really don’t know the long-term effects. There’s just so many questions and every day you read something different.”

    Andriy Petriv is a long-haul truck driver from the Toronto area. He said he and his wife got sick with what they think was COVID-19 shortly after Christmas. While they didn’t get tested, Petriv said they had all the usual symptoms.

    ‘I just don’t see the point’
    To satisfy his curiosity, he said, he recently had an antibody test to see if he had developed any immunity to COVID-19. The test, which is used to determine past infection, showed that he had developed some antibodies to the virus.

    “Since I already had it, I don’t see the point of taking a vaccine. It could be dangerous in some cases and given the fact that I already have antibodies, why should I even take a risk?” he said in an interview.

    “If I have to take it, I’ll take it. I’m not scared of vaccines. I just don’t see the point. Why put something in my body just to have a certificate or something? If you’re not thirsty, why should you drink just to make somebody happy?”

    He said he’s also disturbed by the fact that the U.S. Food and Drug Administration (FDA) has so far only granted the COVID-19 vaccines an emergency use authorization, not “full approval” — a process that can sometimes take years. The FDA has said full approval is coming.

    Health experts maintain that even people with past infections should get a vaccine. Some jurisdictions, however — including Quebec, France, Germany and Italy — have been administering just a single dose to anyone with a confirmed previous diagnosis.

    “While you will receive some immunity from having a previous infection, it remains unclear the duration and breadth of that immunity,” said Dr. Kumanan Wilson, a professor of medicine at the University of Ottawa.

    “It’s uncertain whether being exposed to a previous version or variant of the virus will protect you against new variants as strongly as a vaccine will.”

    Vaccine acceptance is growing

    Shannon MacDonald is an associate professor in the faculty of nursing at the University of Alberta. Before the immunization campaign got underway, she conducted a study on the acceptability of COVID-19 vaccines among the Canadian population.

    She found that, in general, the vast majority of Canadians are not diametrically opposed to vaccines. In fact, fewer than 2 per cent of Canadian parents refuse childhood shots for their kids.

    Knowing little about the shots that would soon be deployed, 65 per cent of Canadians polled for MacDonald’s study said they would get a COVID-19 vaccine as soon as Health Canada approved one for use — a figure she described as “hugely encouraging.”

    The number of willing vaccine recipients has grown steadily since that study was published.

    “Unfortunately, the small proportion are quite vocal and there’s a perception that they’re bigger than they are. I think focusing on people who have really legitimate questions — and when I say legitimate questions, I don’t mean their concerns are necessarily based on facts — is really key,” MacDonald said in an interview.

    ‘Breakthrough cases’ extremely rare

    MacDonald said the best way to convince the hesitant is to show them the data on just how effective the vaccines have been at preventing infection.

    For example, of the 403,149 COVID-19 cases reported in Ontario between December 14, 2020 and July 10 of this year, just 0.4 per cent were so-called “breakthrough cases” — COVID-19 infections in people who had received their second doses 14 days prior.

    About 4 per cent of all cases reported in that seven-month period were people who were partially vaccinated with just one dose. The rest, of course, were unvaccinated.

    As of July 10, fewer than 18,200 of the 10,000,000 people who have received at least one dose so far in Ontario have contracted the virus — 16,358 were infected when they were only partially vaccinated and 1,765 became infected after having two doses.

    In the U.S., the Centers for Disease Control estimates that 97 per cent of the people who have been admitted to hospital recently with COVID-19 are unvaccinated.

    The trust factor

    MacDonald said the very low number of adverse effects should also assure the hesitant that these products are safe.

    “The safety profile has been impressively good,” she said. “You could put the message on a billboard and that might reach some people, but for people who are distrustful of the government, pharmaceutical companies, whatever, they need to hear the message from people that they trust. We have to get the message out there.

    “All it takes is one case in your unvaccinated community and you’re all at risk.”

    According to Public Health Agency of Canada data, there have been only 2,222 serious adverse events reported post-vaccination in Canada as of July 9. That’s just 0.005 per cent of all doses administered.

    Despite these positive indicators, MacDonald said the vaccination campaign will almost certainly hit a wall of entrenched hesitancy.

    A fourth wave of cases might convince the unconvinced that they’re better off with a shot, she said. “You’d hate to wait to see an outbreak to say, ‘See this is what could happen.’ But that might be the case.”

    She said public health authorities should still try to persuade some of the unvaccinated but, at a certain point, those energies might be better spent on getting the partially vaccinated back for that crucial second dose.

    “Let’s focus on them instead of jumping through one hundred hoops to try and get a first dose into people who aren’t interested,” she said.

      More than 5,100 Massachusetts residents have tested positive for COVID-19, despite being fully vaccinated against the virus, and at least 80 of them have died, state health officials said Tuesday night.

      The latest update from the state Department of Public Health comes as coronavirus metrics continue to creep up in the Bay State while the more contagious delta variant keeps spreading in the U.S.

      The so-called breakthrough cases — where fully vaccinated individuals test positive for coronavirus — have so far been rare, but are possible. And should even be expected, per the CDC.

      As of July 17, a total of 5,166 breakthrough cases had been reported to the state DPH. Of those, 272 people were hospitalized and survived. Of the 80 people who died, 23 died without being hospitalized; 57 died following a hospital stay.

      The death toll reflects 1.54% of the 5,166 confirmed breakthrough cases and 0.0018% of the 4,307,361 Bay State residents fully vaccinated as of July 20.
      Health officials insist vaccine confidence should not be shaken, and say these numbers should be put into perspective. The overwhelming majority of the reported breakthrough cases — more than 93% — did not result in either hospitalization or death.

      “I think we have to be cautious but we have to keep it in perspective,” said Michael Curry, the president and CEO of the Massachusetts League of Community Health Centers.

      Tufts Medical Center epidemiologist Dr. Shira Doron says since people have started rolling up their sleeves for COVID vaccines, the Boston hospital has only seen two patients hospitalized for COVID-19 in those who were fully vaccinated. Neither died.

      “What hasn’t changed is that the vaccine is preventing severe disease and death,” Doron said.

      Doron also acknowledged while people are getting nervous hearing about breakthrough cases, she really believes the focus right now needs to be on getting more people vaccinated here in Massachusetts and across the U.S.

      That’s what keeps us safe, she said. Doron also stressed that adverse events associated with the vaccines are extremely rare.

      “You have a small but larger than the gamble you’re taking with the vaccine the chance of ending up in the hospital, in an ICU on a ventilator or potentially dead,” she said.

      According to Curry, the time to prepare is now.

      “We need businesses to urge people to get vaccinated. I think businesses have to make a judgement call and decide where necessary to have a mask on when people come in. I know we’re not in a state of emergency where we once were, but we may need to get back to those strategies. Quite frankly, it saves lives.”

      As the island nation’s chief epidemiologist put it, commenting on the recent exponential growth, the new coronavirus wave is definitely here, for the most part amongst the vaccinated.

      Four weeks after having lifted all of its anti-coronavirus measures, Iceland is now once again slumping into a partial lockdown, the news outlet Iceland Review reported.

      The reason for the U-turn are the dozens of new cases of infection registered daily, a significant number in a country with only 360,000 inhabitants. Iceland is witnessing an increased spread of infection, and the authorities fear that the more contagious delta strain of the virus will gain a foothold in the island country.

      To offset the negative development, starting from Sunday, bars and nightclubs must close at midnight, face masks must be used indoors where people meet, a maximum of 200 people can gather at events, and the one-metre rule will be re-introduced. Swimming pools will remain open, but won’t be allowed to work above 75 percent capacity.

      The restrictions are based on recommendations from Chief Epidemiologist Thórólfur Gudnason, who emphasised that COVID-19 case numbers were rising at an exponential rate despite the country’s high vaccination rates. He also emphasised that “the new coronavirus wave is definitely here”, for the most part amongst the vaccinated.

      As of now, around 90 percent of all residents over age 16 have received at least one dose of the coronavirus vaccine.

      A month ago, on 26 June, Iceland lifted all domestic COVID-19 restrictions, one year and four months after the very first ones were imposed due to the pandemic. The country also loosened border restrictions on 1 July, allowing travellers with proof of vaccination or previous infection to enter the country without testing or quarantine, becoming the poster child of the international media for successfully battling the epidemic with only slight damage to the economy.

      However, infection rates have risen over the past two weeks, and Iceland now reports 371 active cases, up from 60 cases just eight days ago.

      All in all, Island has seen 7,148 cases of COVID-19, with 30 fatalities.

      The figures suggest vast numbers are being classed as hospitalised by Covid when they were admitted with other ailments, with the virus picked up by routine testing.

      Experts said it meant the national statistics, published daily on the government website and frequently referred to by ministers, may far overstate the levels of pressures on the NHS.

      The leaked data – covering all NHS trusts in England – show that, as of last Thursday, just 44 per cent of patients classed as being hospitalised with Covid had tested positive by the time they were admitted.

      The majority of cases were not detected until patients underwent standard Covid tests, carried out on everyone admitted to hospital for any reason.

      Overall, 56 per cent of Covid hospitalisations fell into this category, the data, seen by The Telegraph, show.

      Crucially, this group does not distinguish between those admitted because of severe illness, later found to be caused by the virus, and those in hospital for different reasons who might otherwise never have known that they had picked it up.

      Last month, health officials instructed NHS trusts to provide “a breakdown of the current stock of Covid patients”, splitting it into those who were in hospital primarily because of the virus and those there for other reasons. So far, NHS England has failed to publish this data.

      However, the patterns shown in the leaked figures – with the vast majority of hospital Covid cases being diagnosed after admission, in some cases weeks later – suggest it includes large numbers likely to have been admitted for other reasons.

      The breakdown of daily Covid hospital diagnoses shows that of more than 780 hospitalisations dated last Thursday, 44 per cent involved people who tested positive in the 14 days before hospital entry.

      A further 43 per cent were made within two days of admission, with 13 per cent made in the days and weeks that followed, including those likely to have caught the virus in hospital.

      Experts said the high number of cases being detected belatedly – at a time when PCR tests were widely available – suggested many such patients had been admitted for other reasons.

      Prof Carl Heneghan, director of the Centre for Evidence-Based Medicine at the University of Oxford, said: “This data is incredibly important, and it should be published on an ongoing basis.

      “When people hear about hospitalisations with Covid, they will assume that Covid is the likely cause, but this data shows something quite different – this is about Covid being detected after tests were looking for it.”

      Prof Heneghan urged the Government to publish clearer data, including whether or not the virus was the primary cause of hospital admission.

      “This needs to be fixed as a matter of urgency,” he said, adding that the published data could lead the public “towards false conclusions”, exaggerating the true levels of pressures on hospitals.

      Sir Graham Brady, the chairman of the 1922 committee of Tory MPs, said: “Nearly 18 months into the Covid crisis, it is absurd that data breaking down hospital admissions still isn’t publicly available on a regular basis.

      “Counting all patients who test positive as Covid hospitalisations is inevitably misleading and gives a false picture of the continuing health impact of the virus.”

      Greg Clark, the chairman of Commons science and technology select committee, on Monday night said he would write to Sajid Javid, the Health Secretary, asking him to publish the breakdown on a regular basis following The Telegraph’s disclosure.

      “If hospitalisations from Covid are a key determinant of how concerned we should be, and how quickly restrictions should be lifted, it’s important that the data is not presented in a way that could lead to the wrong conclusions being drawn,” he said.

      “While some of these people may be being admitted due to Covid, we currently do not know how many. And for those who are not, there is a big distinction between people who are admitted because of Covid and those are in for something else but have Covid in such a mild form that it was not the cause of their hospitalisation.”

      The leaked statistics come from NHS daily situation reports, collected by all hospital trusts in England.

      One NHS data expert said the published statistics distorted the true picture, saying: “It creates an impression that all these people are going into hospital with Covid, and that simply is not the case. People are worried and scared and not really understanding the true picture – that is what I find despicable.”

      An NHS spokesman said: “Many patients are admitted to hospital because of their Covid symptoms and complications, which are then confirmed with a post-admission Covid test, and for others they may initially be presymptomatic or asymptomatic.”

      On Tuesday, NHS leaders said the health service is as stretched now as at the height of the pandemic in January and made a plea for extra funding.

      In a letter to the Prime Minister, Chancellor and Health Secretary, NHS Providers, which represents hospital trusts, raised fears the situation could get worse before it gets better.

      They said “very different pressures” – including a “massive backlog” of patients in need of care, high numbers of staff self-isolating or on annual leave, meant the strain on the service now is just as bad as at the start of the year.

      Acute Myocardial Infarction Within 24 Hours After COVID-19 Vaccination

      Introduction

      The United States launched the COVID-19 vaccination campaign in mid-December 2020 as a measure to contain the pandemic. While there are limited data on the risk of thrombotic events related to the vaccines currently in use in the United States, there have been reports on thrombotic events after COVID-19 vaccines in Europe recently. We report two cases of acute myocardial infarction with onset less than 24 hours after the first dose of a COVID-19 vaccine in the United States. Early recognition of this acute cardiovascular condition could be challenging, as injection site soreness may lead to ischemic symptoms being overlooked and result in delayed presentations. This report should in no way diminish enthusiasm about vaccination.

      Patient 1

      In the first case, the patient experienced gradual onset of left shoulder pain within one day after her first dose of COVID-19 vaccine (mRNA-1273) that later progressed to left-sided chest pain. She initially attributed her symptoms to the vaccine. She presented to the emergency department (ED) approximately two hours after chest pain (Table 1). Bedside cardiac ultrasound showed estimated left ventricular ejection fraction of 50% and hypokinesis of the anterolateral and inferolateral walls. The patient tested negative for SARS-CoV-2 by PCR on two separate nasopharyngeal aspirates. Emergency coronary angiography revealed an occluded proximal segment of the left circumflex (LC) artery with a globular thrombus and TIMI 0 flow (Figure 1 & Video 1).

      Emergency percutaneous coronary intervention (PCI) was performed to the LC artery. A workhorse wire (Asahi 0.014” Sion Blue) was used to cross the lesion. The culprit lesion was pre-dilated with a 2.5 mm?×?12 mm semi-compliant balloon. A 4.0 mm?×?18 mm everolimus-eluting stent was placed in the proximal segment of the left circumflex artery and was optimized with a 4.0 mm?×?15 mm non-compliant balloon. The final angiogram showed an excellent result with TIMI III flow (Video 2). The patient was free of chest pain after PCI. Formal cardiac ultrasound one day after PCI showed a left ventricular ejection fraction of 60% with only mild hypokinesis of the anterolateral and inferolateral walls. She was discharged home one week after admission.

      Patient 2

      The patient in the second case presented with 4 days of chest and shoulder pain. Symptoms started the night after he had his first dose of COVID-19 vaccine (mRNA-1273). He woke up from sleep with sharp, intense chest pain that radiated to the jaw. Symptoms slightly improved but remained for 4 more days, and he finally presented to an outside hospital. He also reported injection site pain from the vaccination but was otherwise asymptomatic. At the emergency department, he was found to have elevated troponin with no significant ECG changes. He was then transferred to our center, and a CT coronary angiogram revealed complete occlusion of the LC artery. In view of this finding, he was transferred to the catheterization laboratory (Table 1).

      Ultrasensitive troponin-I at the outside hospital was 9200 ng/L (normal: 0 – 47 ng/L). After transfer to our center, the high-sensitivity troponin-T rose from 723 ng/L (normal: 0 – 14 ng/L) to 1763 ng/L. Patient tested negative for SARS-CoV-2 by PCR first at the outside hospital and tested negative again at our center. Cardiac ultrasound showed left ventricular ejection fraction of 60%, with hypokinetic inferior and inferolateral walls. CT coronary angiography showed a large amount of non-calcified plaque in the proximal LC artery, resulting in complete focal occlusion just proximal to the origin of first obtuse marginal artery. Coronary angiography in the catheterization laboratory revealed a 90% stenosis in the proximal LC artery with TIMI 1 flow (Figure 2 & Video 3). The rest of the coronary arteries were free of significant disease.

      Emergency PCI of the LC artery was performed. A workhorse wire (Asahi 0.014” Prowater) was used to cross the lesion. The lesion was dilated with a 2.5 mm?×?12 mm semi-compliant balloon. A 3.0 mm?×?18 mm zotarolimus-eluting stent was deployed to treat the lesion and was optimized with a 3.5 mm?×?12 mm non-compliant balloon. The final angiogram showed an excellent result with TIMI III flow (Video 4). The patient had no more chest pain after PCI and remained hemodynamically stable. He was initiated on guideline-directed medical therapy and discharged home two days later.

      Discussion

      We report two cases of AMI that occurred within 24 hours after the first dose of COVID-19 vaccination. The angiographic features of both cases were suggestive of acute thrombotic events as the underlying mechanism. Both cases were notable for the variable degree of delay in presentation, as patients attributed the chest pain to local effects of the vaccine injection. Both patients received the same COVID-19 vaccine (mRNA-1273). Acute myocardial infarction was not reported in the original study of this vaccine.1), 2), 3) There was no significant difference in thrombotic events between the vaccinated group and placebo group.4) A search yielded only a single case report of AMI early after injection of the same vaccine.5) On the other hand, as of March 10, 2021, among the 5 million people vaccinated with the ChAdOx1 nCoV-19 vaccine in the European Economic Area, 30 cases of thromboembolic events have been reported.6) It is worth noting that the ChAdOx1 nCoV-19 vaccine triggers immune response by nCoV-19 spike protein,7) while the mRNA-1273 vaccine induces antibody response with a lipid-nanoparticle-encapsulated mRNA.

      As the COVID-19 vaccination campaign moves toward a new phase around the world and vaccines become available to the general public, more data will be available on safety and potential side effects. More data on the nationwide trends of myocardial infarction or other thrombotic events before and after vaccination launch should be carefully obtained and interpreted before commenting on any possible causal relationship. Furthermore these two case reports should not dampen enthusiasm for vaccination but should raise awareness of the possibility of myocardial ischemia in patients having shoulder pain after vaccination.

    The mainstay of the recent flare-up is the Turkish offense against Kurdish fighters in both Iraq and Syria. Turkey claims that these attacks are in response to attacks carried out or planned by elements of the “PKK”, which Turkey has been fighting for 37 years inside and outside the country. However, for Iranian-backed militias in Iraq and Syria, Turkey’s actions are nothing less than a full-blown transgression against their sovereignty and territorial integrity. Consequently, an Iraqi militia backed by Iran has now vowed to “cut off the legs” of Turkish soldiers if they came to “occupy” any part of the country.

    In China, Scrutiny is not a word in Communist Party’s dictionary. The CCP has been undeterrably calling the shots in the country of 1.4 billion since its inception. In Hong Kong too, the tyranny has engulfed once-thriving democratic institutions after the so-called National Security law was brazenly imposed on the city last year. Despite the vehement crackdown on pro-democracy fractions in the city, the ray of hope is still visible to the disenchanted Hongkongers. Much to the dismay of the Communist Party and its officials, the independent courts in Hong Kong are still giving a tough time to the police and CCP officials working overtime to enforce the draconian law at the behest of a megalomaniac Xi Jinping.

    As reported in Reuters, a Hong Kong district court on Thursday sentenced seven men to between 3-1/2 and 7 years in prison for their role in a 2019 attack on pro-democracy protesters, reporters, and bystanders at a train station at the height of anti-government protests. The sentence has marked the first conviction of any of the infamous white-shirt attackers who were accused of ruthlessly beating pro-democracy protestors last year. While handing the sentences, Judge Eddie Yip described the crimes as “mob justice which caused panic among the general public.”
